Client Care Summary
Female client will need hands-on assistance with activities of daily living. Our client will need assistance with personal cares, housekeeping and laundry tasks, and community participation. Client needs assistance with going up and down stairs.
ICLS Job Description
Provides individualized support to clients in their home and community settings. Assists with activities of daily living, personal care, household tasks, community involvement, and skill development while promoting independence, dignity, choice, and safety. It includes the following service categories:
Active cognitive support
Adaptive support service
Activities of daily living (ADLs)
Household management
Health, safety and wellness
Community engagement
Requirements
• Must have valid identification.
• Must have valid car insurance.
• Must be at least 18 years of age.
• Must have reliable transportation.
• Be able to lift 50 pounds or more.
• Pass a Department of Human Service background check.
• Provide tasks as required in the individual’s 245D Care Plan.
• Complete online CDS modules 30-days after starting with client and annually.
• Okay with repeated bending, stooping, etc. as necessary to meet the needs of individual clients.
