245D Basic Waivered Services are designed to support the person in maintaining independence, safety, dignity, and quality of life in their home and community. Services are individualized based on the person’s assessed needs, preferences, goals, and authorized service plan. Support may include assistance with daily living activities, personal support, community participation, health and safety needs, household routines, skill development, and other approved supports intended to help the person remain as independent as possible.
