Sincere Home Care staff and clients may use this form to request for their monthly PPE bags. Please note that there is a limit of one bag every 30 days per individual. Each PPE bag will contain a hand sanitizer, gloves, masks, paper towel roll, alcohol wipes, pen, posted notes, medical gowns, and face shields if applicable for client care.
